"Your Story" Submission Form

There are amazing patient stories happening in our clinics and communities every day as a result of the excellent care ATI provides to our patients. To gather these stories and promote them through various channels (ATI TV, atipt.com, social media, etc.), we have created an easy online form to submit your photos and stories.

Instructions:

1. Please complete our waiver form, Media/Public Relations Authorization for the Use and Disclosure of Protected Health Information. You can also attach a scanned waiver to the form below.
2. Submit stories and photos using the form below – remember, a picture is worth a thousand words! (Please do not submit photo collages - send photos individually, as they may need to be resized appropriately for various media channels, etc.)

Note: Submission of material does not guarantee use, and ATI Marketing reserves the right to deny publication of submitted images/content on Facebook, Twitter, LinkedIn, ATIpt.com, ATI TV and other forms of marketing media. All submissions that contain patient images or information must be accompanied by a Media/Public Relations Authorization for the Use and Disclosure of Protected Health information form before use can be considered.  Please click on and complete the form and submit it with content below. Patients may submit release forms seperately, as long as they are identified by name in the photo submission.
